Qsource ESRD Networks’ Data Services Department staff provide technical assistance to dialysis providers and transplant programs in each of our five states. We assist with ESRD Quality Reporting System (EQRS) data entry questions, forms compliance, such as 2728, 2746, and the annual 2744 survey, and general patient reporting questions.
