Qsource ESRD Networks act in partnership with nearly 700 providers of Medicare-certified outpatient chronic dialysis services and more than 20 transplant centers within the States of Illinois, Iowa, Kansas, Missouri, and Nebraska. Through collaboration with this stakeholder community, we strive to assist dialysis providers in their efforts to improve quality of care and quality of life for more than 38,000 ESRD patients.
Qsource ESRD Networks are committed to assisting dialysis clinics improve quality and enhance patient care. It is our goal that our Quality Improvement activities will be an effective addition to your ongoing quality improvement measures and that you will be able to engage both patients and staff.
Qsource ESRD Networks are authorized to receive, investigate, and process grievances related to the quality and safety of care received by patients in Medicare-certified dialysis or transplant facilities in the states of Illinois, Iowa, Kansas, Missouri, and Nebraska. The Networks may serve as an investigator, facilitator, referral agent, coordinator, and/or educator.
Qsource ESRD Networks are a resource for facilities when they have questions about how to perform tasks in the ESRD Quality Reporting System (EQRS), or need assistance in correcting EQRS data elements. Networks 10 and 12 work directly with facility staff to increase their EQRS data completeness performance rates.
