Driving Improvement

It takes a village to drive quality improvement. Ideas can spread and impact the lives of thousands. As a community coalition member, you join peers from across the spectrum of care to address the most pressing local healthcare issues.  

Together, we are building a network of stakeholders dedicated to strengthening the voice of patients, providers and communities wherever decisions shaping the future of those living with End Stage Renal Disease are being made.

What is a Community Coalition?

Community coalitions function as bodies of diverse stakeholders within a community who are dedicated to defining a healthcare issue within the designated community. Community coalitions focus on: 

  • producing a root cause analysis to identify areas for improvement,  
  • committing to work as a group to achieve quantitative aims,  
  • and implementing specific actions tied to the identified root causes that are designed to improve healthcare outcomes within the community.  

How Does It Work?

Community coalition building makes efficient use of limited resources. By connecting the community together, our collective efforts can achieve tangible, data-driven change. Members share knowledge, insight and aggregate data in order to evaluate and improve healthcare quality in their area.  

Who Should Join?

We encourage all stakeholders from across the continuum of care to join, including but not limited to: 

  • Empowered patients 
  • Nephrologists 
  • Primary care providers 
  • Dialysis facility staff (including local and regional management) 
  • Nursing home staff 
  • Transplant surgeons 
  • Transplant center staff  
  • Organ Procurement Organizations (OPOs) 
  • Psychologists/Psychiatrists
